Step 1
Call us, just to chat! We’ll explore motivations, concerns, things to consider, brainstorm, and collaborate! (10 to 60 minutes)

Step 2
Schedule an office visit or in-home visit to complete preliminary needs assessment by Nurse Nell!
Budget review, clinical and medication history, location preference, family dynamics, community amenities (preferences for activities, dining, care levels, off site outings, on site religious, pet requirements, fitness amenities, other considerations. (45 minutes to 90 minutes). Assemble timeline. Target a move date!
Care Plan Development. You will be provided with a professional written summary of our geriatric assessment findings. This will also include our specific recommendations based upon those findings.
Step 3
Research by our internal staff for community tours, inspection, evaluation of various communities, pros and cons. Assemble and review comparison data.
Best five! Based on your criteria, the needs assessment and care plan development, we will develop a list of the Top 5 Senior Communities that would be the perfect match! Having worked with, and inside many of these communities in Metro Denver we’ll offer first-hand knowledge from our own experience.
Step 4
Schedule and complete community tours. Assemble and review comparison data. Family members may wish to schedule a time to visit and join the SLOCO Senior Care Advocate for tour and review of these properties.
Step 5
Decision time! Our Senior Care Advocate will guide you every step of the way to a decision. Your choices will include the pros and cons of each community, as well as a summary of discounts, concessions, cost review and stipends offered by each community.
Prior to moving into the senior community, the resident may be required to have an assessment by the community; a care plan for the resident will determine the cost of care services.
Step 6
Move Coordination. A Senior Care Advocate can provide full service coordination of a move, through the enlistment of a community partner that can aid support to downsize & de-clutter, helping the senior and family decide what’s really important in this emotional process and develop a punch list, and an action plan to handle all aspects of a move. Families that have a Senior Care Advocate to assist and coordinate resources have a much smoother transition.
Step 7
FREE Transitional Assistance
On the day of the move and for the entire day after, SLOCO will provide a TRANSITION AMBASSADOR FREE of charge! This trained very experienced caregiver will provide the NEW resident with companion assistance through-out the day, make introductions to other residents, community staff, escorts to dining and activities, and assistance with navigating the NEW environment for the NEW resident. This happens FREE of charge! No other placement agency will do this FREE of charge!
